Request to reconsider your concession application

If your application for a Blue Badge or a Disabled Person’s Freedom Pass was not approved, you can fill in this form to ask us to review that decision.

This form usually takes about 10 minutes to complete.

You will need:

• the reference number from your original application
• digital copies of all documents you submitted before and any new ones

We follow guidance from the Department for Transport on who we can issue Blue Badges and Freedom Passes to. You can check the eligibility conditions here:
