Criminal Record Self Declaration Form

It is a Council requirement that employees in specified post complete an annual self-declaration relating to criminal convictions incurred since their previous criminal record checks/disclosure and barring service (DBS)/annual self-declaration.

These posts fall under the provisions of the Rehabilitation of Offenders Act 1974 and Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 which exempts them from the original Acts non disclosure of spent convictions.

Please complete form and submit. If you fail to complete the form, this will be regarded as a disciplinary matter for staff and in the case of volunteers, will mean that you can no longer continue in your existing role.
