Application for a Personal Licence

Before completing this form, you must have:

1. 2 photographs of yourself, one of which is endorsed as a true likeness of you by a solicitor or notary, a person of standing in the community or any individual with a professional qualification.


2. Licensing qualification you hold or proof that you are a person of prescribed description.


3. A valid Basic Disclosure Check (DBS) which is dated within the last 28 days or my 9-digit share code, along with the last 6 numbers of your certificate number so we can check your result.


4. Disclosure of convictions and declaration. Please download, sign and submit with the application.


5. Proof of your right to work in the United Kingdom or my 9-digit share code issued by the Home Office online right to work checking service.
