Application for alternative provision 2023-2024

If the child has an EHCP please do not complete, contact your EHCP Co-ordinator.

Please use this form to apply for alternative provision including interim provision for pupils with medical needs. All applicants must provide a risk assessment. For all medical applications, the separate medical evidence form must be completed.

Please check that you have provided as much detail as possible, this will support a full assessment of the child’s education needs.

At the end of the form you will be required to upload key documents such as medical evidence forms, individual health care plan and pupil’s attendance record. It is advisable to have these ready to upload.
