Parent carer needs assessment form

This form is for parents or other primary carers that look after a disabled child under the age of 18 in Southwark.

If you are caring for an adult over the age of 18, find out who to contact for a carer's assessment.

Please complete this self-assessment to tell us more about your caring responsibilities and how this impacts you and your family.

The assessment process is managed by Southwark's Disability Hub on behalf of Southwark Council. The Disability Hub is delivered by national charity KeyRing.

Your completed self-assessment will be shared with the Disability Hub. They will follow up with a phone call or meeting to discuss the contents of your self-assessment and next steps.

If your child is open to the All Age Disability Service then your self-assessment will be shared with your assigned worker so they can pick this up with you directly.
