Parking - Disabled Parking Bay Application

Please ensure you have read through the guidance notes before completing your application.

To qualify for a disabled bay, applicants must be in receipt of either:

• Disability Living Allowance (DLA) Higher Rate Mobility OR
• Personal Independence Payment (PIP) mobility component with 8 points or more for moving around

All other applicants including Blue Badge holders who are not in receipt of the above benefits may be referred for an Occupational Therapy Assessment.

Document check list

Before completing this form please ensure you have copies of the following documents to attach to your application:

• A picture of both sides of your Blue Badge
• A photograph of your preferred bay location (please note a Google maps screenshot is not sufficient)
• V5 or insurance document that confirms the vehicle is registered to the address

Please note it normally takes up to a year to install a disabled bay.
